Medication Reconciliation Reflection
The patient that I had was a 71 year old women, when looking at the medications that she was taking there did not seem to be any safety concerns. The patient knew why they were taking the medications as well as what they were able to do. There were some general concerns with them taking omeprazole as they are older and with age this drug can increase fractures but as this patient doesn’t have a history of falls, right now she is fine. The patient knew why they were taking these drugs as well as how much of the drug they should be taking. This is important because if they are taking two medications that do the same thing, which they aren’t, can become harmful. This is also good to know incase they get hurt and need to tell someone what they’re on. All the medications that the patient are taking are to help improve their health, with no serious interactions that they need to worry. The patient should know the side effects of the drugs as well as what would happen if another drug was to interact with them.
